Not a one-off (because there are two of them), but Mercedes-Benz Advanced Design Center in Japan once made a pair of second generation smart crossblades.

The Ferrari P4/5 was the first to come to mind but here are a few others:

The Fenomenon Stratos. I actually saw this thing in person about 10 years ago. I’ll see if I can dig up the photo on a bit.

because it’s a Prodrive one off.
or
because it’s the worst GTI ever built in every measurable but it has a W12 so none of that other stuff really matters.

Cadillac Cien. Really any of Cadillac’s concepts since 2000 or so have been awesome.

Found it! Fenomenon Stratos at the start of the Gumball 3000 in May 2005. (I wasn’t planning on going to this, I just stumbled on it and fortunately had my camera with me.)

I can’t decide. I have several favorites.
Candidate 1: Bugatti Type 41 Royale Esders Roadster.
The real one doesn’t exist in this form, but a replica has been built from unused spare parts. The original didn’t have any front lights, as Mr Esders wasn’t planning to drive during night time.
Candidate 2: Lamborghini Countach LP400 2nd prototype
With a quick glance it looks like a normal early production version, but it does have many things separating it from the rest. The two piece side windows are the most apparent difference.
Candidate 3: Alfa Romeo Carabo
Like Countach, Carabo was also designed by Marcello Gandini. They also share several design features. Extremely weird colors are one thing that was unique to Carabo.

It would be pretty much impossible for me to choose a favorite, so I’ll just list some of my favorites.
Ferrari SP12 EC - Love it or hate it, I personally love Clapton’s one-off 458 based 512 BB homage. The only thing that would have made it better would have been if it was an homage to the 365 GTB/4 Daytona.
Pininfarina Sergio - The most fitting tribute to the late Sergio Pininfarina that could have been imagined. Though a limited series of them were built upon the 458 Speciale chassis, the original concept remained a one-off because it was distinctly based on the chassis of the 458 Challenge race car.
Isuzu VX-02 - While not as completely unique as the Sergio or SP12 EC, Isuzu’s VX-02 and VX-04 (a 4-door version, something Jeep would implement themselves upon the introduction of the JK Wrangler) showed off what could have been if the VehiCross expanded into a line of variants like the Rodeo.
Specifically, the VX-02 was called an off-roadster, taking an idea put forward by Suzuki and their X90 a couple years before, and taking it even further by ditching the roof altogether, with a solid speedster style panel behind the seats. Essentially making it a little more akin to the production version of the Jaguar F-Type Project 7 than the X90, which, in retrospect...was pretty awesome.
Lamborghini Asterion LPI 910-4 - This car, almost completely Lamborghini’s equivalent to the Jaguar CX-75, enthusiasts had high hopes for a high performance grand tourer from the brand of the bull, after astonishment after astonishment with cars like the Porsche 918, McLaren P1, and LaFerrari.
Sadly, the Asterion got canned in favor of a yet-to-be-produced production version of the Urus concept. From a profitability standpoint, the Urus does make more sense, being a profit filled SUV. Still, after the successes of the 918, P1, and LaFerrari, many enthusiasts long for what could have been if this beautiful, high tech beast had been built instead.
Rinspeed sQuba - Inspired Bond’s Esprit submarine, Swiss company Rinspeed took a Lotus Elise and turned it into an open top roadster for above and below water, allowing the driver and one passenger to feel like a mix of James Bond and
Joe Walsh
. The electric powertrain also makes it sort of like an amphibious Tesla Roadster.
Ford F-150 RTR - America’s best selling truck for 40 straight years has been the subject of many one-offs over the years, but none are more epic than Vaughn Gittin Jr’s RTR F-150, also known as the ‘ultimate fun-haver’.
Renault Twizy F1 - While not really much faster or better handling than the standard model, it was pretty awesome for Renault Sport F1 to put an F1 inspired kit on their diminutive electric single seater.
Mazda Cosmo 21 Concept - In 2002, Mazda paid tribute to one of their most coveted creatons, the Mazda Cosmo Sport with this stunning MX-5 based concept. Yes, it was packing a rotary engine: the Renesis 13B that would later appear in the RX-8.
